On-Site Care Pricing Models

FULLY EMPLOYER FUNDED
An annual fee is contracted and the employer is invoiced 1/12 of that amount each month. This package includes on-site care hours, off-hours support, employee education, ergonomic services and outcome reporting.
EMPLOYER FUNDED: BASE PLUS PERFORMANCE
Employer is invoiced contracted base fee each month. In addition, there is a contracted performance bonus. The performance bonus is tiered based on the program’s ROI. The employer pays half of the anticipated ROI bonus and the balance is calculated at the end of the year.
EMPLOYER PAY PER VISIT (Tiered, Capped)
The employer is invoiced each month based on the total number of new patient visits and regular office visits. Additionally, there is cost per visit tiering and a monthly cap set. The cost per visit is less than the national average cost per visit, reflecting administrative savings. Additional services are billed separately.
EMPLOYEE CO-PAY WITH EMPLOYER FUNDING
Option A
Employer is invoiced each month based on the total number of new patient visits and regular office visits. The employees have a co-pay per visit.
Option B
Employer is invoiced each month for a contracted amount and employees have a co-pay per visit.
